Payment Plans and Fees

Granted that you provide insurance information, we will bill primary and secondary insurances for you. If you have one of our accepted insurance companies, it is required that you pay your estimated deductible or co-payment before your procedure can begin. Patients are responsible for their procedure payment.
Payment forms that are accepted at our outpatient facility include: cash, personal checks and credit cards. Once payment is received and verification of insurance is determined, then we will bill your insurance company for you.
